Chino
Hills Little League require each player to have a Medical
Release on hand. Parents are responsible for filling out
this form and turning it to your team manager. Managers are required to
have these forms at all times during games in case of an emergency
should arise. Players cannot participate in any games if they have not
filed their Medical Release.
